Core Wellness Assessment
Initial wellness assessment including a comprehensive metabolic panel, lipid Panel, thyroid panel, Vitamin D and hemoglobin A1C.
Sick care visits
Includes 2 monthly virtual visits as needed for minor issues such as colds, UTIs, sinus infections, rashes and more.
Medication Refills (Non-Controlled)
Includes 2 visits for blood pressure, thyroid, and cholesterol medications refills. Blood work can be ordered when needed to ensure safe and effective treatment.
Monthly Secure Messaging
You will be able to communicate with your provider for brief consultations or updates, receiving continuous care between appointments through a secure portal with priority response.
